New Patients / Review Patients:
1. Appointments booked online cannot be cancelled by the patient.
2. Patient shall report at the concerned counter at the Hospital 30 mins prior to the appointment time slot with a the confirmation email/SMS and goverbment approved Photo – ID. Failure to report before 30 mins with required documents will be treated as no show . No refund will be made in the event of the patient not turning up and the appointment is treated as no show.
3. Appointment slots/time are only provisional and is subject to change based on the work condition of the concerned doctor on the particular appointment date. Though all efforts will be made to see the patient within the appointment time slot, does not assure any patient that the appointment would be time bound.
4. Change of doctor not allowed for appointments booked online.
5. For Appointments made prior to 48 hrs of the appointment date , Change in appointment date will be allowed only once if request is made 24 hrs prior to the Appointment date through the web appointment helpline: between 9am – 4pm. For appointments made within 48 hrs of the appointment date, no request for change in appointment date will be entertained.
6. and the concerned doctor always have the right to cancel the appointment at anytime. In the event of the appointment being cancelled by or the Concerned Doctor, the fee paid will be refunded only to the account from which the same was received and refund from such cancellation cannot be used to book another appointment. The time for refund will be 21 days from the date of cancellation.
7. and the concerned doctor reserve their right to rescheudle any appointment made online. In the event of any rescheduling, the same will be intimated to the patient through SMS or Email provided by the patitent at the time of online registration. The patient shall be entitled to accept the rescheduled date/time/slot or seek for refund.
8. In the event of canellation/ rescheduling of the appointment by , will be liable only to refund the collected fee and will not be responsible for any other cost, expenses or damages.

11. Please provide at-least One Government Related ID proof and Confirmation printout/SMS when you present yourself at the Hospital.

14. Please do not fill up the new patient form twice if you have to visit another Doctor.
